Hello Kevin, interesting question. As Harth is considered the primary Karuna symbol, if you only wanted to send one symbol distantly, then this might be the best one. However, when I personally send Karuna distantly, I first use the Usui Distant to make the connection then tend to send all of the Karuna symbols if I am sending to a group of people. If I was sending one on one, then I may possibly be guided to only use one or two Karuna specifically for that person, but again, in the past I've tended to use the whole eight Karuna One and Two symbols as they all go so well together and work on so many different levels. I've been doing a lot of group Reiki drum journeys lately for groups of ten or more people and tend to now incorporate Karuna symbols whilst drumming. I've found that intuitively the symbols needed for specific concerns individuals in that group are working on, just tend to come through to me whilst drumming, and the same would apply whilst doing distant healing. You'll be guided which ones to use. So to sum up, I'd recommed Harth if you just wanted to use one, every symbol if you want to cover all bases, or alternatively, just go with the guidance which comes through (which is always the better option!).
